If somebody requested that you fill out a Form, and you’re not sure why it says “GoFormz” on it, this article aims to answer your questions.

Fill out Forms on either your personal computer or mobile device. To fill out a Form. . .
First, select a field to enter the requested information.
A field typically corresponds with each piece of information requested on a document. But, sometimes it may just be a rectangle labeled with the information being requested.
Next, review each page or section of the requested Form to verify that they’re all completed.
Finally, select Submit. You’ll navigate to a confirmation page.
If you provided your email, you may receive a PDF version of your Form via email.
